Prerequisites: Phlebotomy with 80% academic score and CPR/BLS.
Requirements: CPR/BLS card, physical exam, drug screen, 2-MMR, 2-Varicella, Tdap within the last 10 years, 2 negative TB skin test within the last 12 months and Hep B series or waiver.
